About the TJN
The Total Joint Network is a partnership of healthcare organizations that work together to refine the healthcare for patients with an orthopaedic problem. The first partnership was formed in April 2005 when 23 healthcare organizations came together to improve the system for patients who were undergoing primary hip or knee replacement surgery by implementing a new model of care. The partnership was made up of organizations from across the continuum of care including acute care hospitals, inpatient rehabilitation hospitals and Community Care Access Centres (homecare). Since this initial project the partnership has worked to define best practice for bilateral and revision knee and hip replacements. In December 2006, the partnership expanded to 35 organizations to look at the care of patients who have sustained a hip fracture. A new model of care was to improve the system of care for patients following a hip fracture and helping them to return home.
The Total Joint Network leads system change by working with each of its partnering organizations in a shared vision of:
- Integration – facilitation of best practice care at all of the patient’s points of contact to create an integrated seamless care model
- Capacity – building capacity where it is required across the system to meet the patient’s needs
- Standardization – promoting best practice as identified through literature, consensus building and through the development of clinical pathways
- Leadership – dedicated administrative and clinical leadership
- Communication – communication both internally within organizations and between partners to provide consistent messaging to patients and families as well as improved problem solving
- Team Work – working together within organizations as well as understanding the roles of our colleagues throughout the healthcare system
- Accountability – working towards agreed upon targets and tracking of performance through monthly reporting
- Innovation – system redesign and internal process redesign to facilitate new practices.
Funding
The Total Joint Network is funded by the government through the application for one time funding for each individual project.
