The Care Maps have been developed by the partnering organizations and reflect best practice for patients through the continuum of care. Principles for Repatriation between Acute Care & Inpatient Rehabilitation

The Principles for Repatriation between Acute Care Hospitals/Units and Inpatient Rehabilitation Hospitals/Units clarify expectations in using the new integrated model of care for patient who have experienced a hip fracture. The principles identifiedl facilitate the best use of acute care and inpatient rehabilitation beds within the GTA and ensure the transfer process is streamlined to maximize the inpatient rehabilitation services available to patients.

Patient Education Brochures

Brochures provide patients with information about the Hip Fracture Integrated Model of Care and are available in black and white and colour. Each hospital is responsible for printing their own copies of the brochure to provide to patients. If a hospital requires the original word version please contact the TJN Project Manager, Janet McMullan.
